Providing Evidence of NHS Entitlement

Providers and suppliers of NHS services require to see evidence of a claimants’ eligibility, in addition to a photo of both sides of the optical voucher, before an order is processed. This is all part of the NHS’s commitment to reducing fraud.

The table below lists the evidence that we need depending on how you qualify for eligibility. If you are unsure on how to obtain evidence of your eligibility, contact the relevant organisation.

You are entitled to an NHS Optical Voucher if:

You are aged 16-18 and in full-time education
What you’ll need to show us:
A letter or other document from your education establishment stating you are a full-time student, or a current Child Benefit order book

You or your partner receive Universal Credit and meet certain criteria
What you’ll need to show us:
A letter of entitlement from your Jobcentre Plus office

You or your partner receive Pension Credit Guarantee Credit
What you’ll need to show us:
Your order book and a letter of entitlement from your Jobcentre plus office

You or your partner receive Income-Based Jobseeker’s Allowance
What you’ll need to show us:
A letter of entitlement from your Jobcentre Plus office

You or your partner receive income support
What you’ll need to show us:
Your income support order book or a letter of entitlement from your Jobcentre Plus office

You or your partner receive Employment and Support Allowance
What you’ll need to show us:
A letter of entitlement from your Jobcentre Plus office

You or your partner is receiving tax credits and meet certain criteria
What you’ll need to show us:
Your valid NHS tax credit exemption certificate

You are named on a valid HC2 certificate and are on low income
What you’ll need to show us:
Your NHS HC2 certificate

You might be entitled for an optical voucher if you are named on a valid HC3 certificate
What you’ll need to show us:
Your NHS HC3 certificate
